All player adds and drops are processed through
Service Providers, INC. There is a service charge of $20.00 for all new player passes issued ($10.00 for inter-club transfers) and $10.00 for all coaches passes that are submitted after the initial team roster registration. This will apply to mail in or walk up service. Checks should be made payable to Service Providers, Inc. When mailing your forms for processing please send them to the PO Box listed below and not to the street address.
