This IWSL all teams coach pass will be valid for 3 years to coach any teams in the league. 
It is a credit card size pass and you must have an A thru E coach’s license to apply. This
coach’s pass takes the place of all other IWSL issued coach’s passes and will be the only one
issued to and in the possession of an individual. It is valid for IWSL league play, tournaments,
and State Cup games below the regional level.

DIRECTIONS:

- Complete the application information.

- Agree to abide by league rules.

- Upload a passport style photo. 

- Include a copy of your A thru E coach license if not registered in our database already. 
   
- Submit any and all current season coach’s passes issued to you.

- Include a check for $15.00 payable to Service Providers and mail to 
   Service Providers, PO Box 68971, Schaumburg, IL 60168.
